Portal Hypertension and Anorectal Varices (Not Hemorrhoids — an Important Distinction)
Anorectal varices are sometimes casually called “hemorrhoids” — even in some clinical writing — but research has established that they are a genuinely separate condition, with a different cause, different risk profile, and different treatment. When pressure in this system rises, blood is rerouted through alternative pathways, including veins around the rectum and anus, causing those veins to dilate — this is an anorectal varix.
Why this genuinely differs from a hemorrhoid
Research specifically studying this question in people with cirrhosis found that hemorrhoids occurred independently of anorectal varices, and that the presence of either wasn’t related to the severity of portal hypertension — evidence that these are separate conditions with separate mechanisms, not two names for the same thing. Hemorrhoids arise from the normal anal cushion tissue described in Why do hemorrhoids happen? — a local, structural phenomenon. Anorectal varices arise specifically from rerouted blood flow due to portal hypertension — a consequence of underlying liver disease, not local anal tissue changes.
Why the distinction matters clinically
Both can cause rectal bleeding, and on casual exam, they can be difficult to tell apart even for clinicians — careful examination matters specifically because misdiagnosis can lead to the wrong treatment. Anorectal variceal bleeding, when it happens, can be more severe and harder to control than typical hemorrhoidal bleeding, because it stems from the same high-pressure venous system implicated in serious complications like esophageal variceal bleeding. Standard hemorrhoid treatments (rubber band ligation, for example) aren’t appropriate for varices and can be risky given the different underlying blood flow; treatment instead focuses on managing the underlying portal hypertension, alongside specific interventional techniques when active bleeding occurs.
